October 6 – November 5, 2012, 4:30 p.m.
NEW HEALTH CARE PROVIDER – ANTHEM!
You must actively enroll in this benefit if you wish to have health care coverage with HCPS in 2013.
There will be NO rollover or continuation of any current health care coverage after December 31, 2012.
Now is your opportunity! Open Enrollment is the period of time each year to review your benefits and make elections that are best for you and your family. Remember, that you will not be able to cancel, change or add to your coverage once you have made these decisions until the next Open Enrollment period. Exceptions to this rule are only allowed with a qualifying event or status change.
To make benefit elections or changes you must use EMPLOYEE DIRECT ACCESS (EDA). Any changes or elections in coverage will become effective January 1, 2013 and must be made through EDA by 4:30 p.m. on November 5, 2012.
